All band and color guard students must get a physical and submit the online Athletic Clearance Forms to the NHHS Athletic office in order to participate in Band Camp and Marching Season. 

 

The Mandatory Athletic Clearance button below will take you to the Sailor Athletic page.  On that page, scroll down and see a few links for Instructions on using Online Clearance, Clearance Forms and Athletic Clearance Website.  It will explain the new online process started with 2019 school year where students medical forms and insurance cards have to be uploaded onto their website, and then print the signature page and turn that into the office.  Their website will explain the details.

The Permission to Carry Medication ... button will take you to a page where you need to print and have your doctor fill out BOTH forms, and return them to the nurses office.
